2025 Foundation Grant Application
Completed application packages must be electronically submitted no later than July 31, 2025, at 11:59 PM.

Neccessary Documentation For Grant Application
Before you start filling out the online form:
1. You will need one of the following to upload in the "Applicant Information" section:
- Form 990
- Audited Financial Statement
- Wisconsin Department of Revenue Financial Report (municipalities)
- Annual financial statement
2. Depending on your type of project and your alternate funding, you need to submit one or more of the following:
- PDFs of Bids - Two Bids Per Service (ie. plumbing, electrical, new furnace).
- Offer to Purchase
- Commitment letters for other sources of funding
3. You should have the following document prepared for upload in the "Project Budget" section:
4. You should also comple the following document which authorizes your submission of the application. This will be uploaded in the "Submit Form" section.
Important: Please Sign the PDF Using a Downloaded Copy
To ensure your signature is properly saved and recognized, do not sign the PDF using your browser’s built-in preview tool (such as in Chrome, Firefox, or Safari). Instead, please follow these steps:
- Download the PDF file to your computer:
- Right-click the link or download button.
- Choose “Save As” or “Download Linked File As…”.
- Save it to a location on your device where you can easily find it.
- Open the downloaded PDF using a PDF application, such as:
- Adobe Acrobat Reader (free and widely used)
- Preview (on Mac)
- Or another trusted PDF editor/viewer
- Sign the PDF within that application, then save your changes.
- Return or upload the signed PDF as instructed.
